How does this work?
Simply select the mode in the battle royale lobby (it should be a square, but it is also there) and then get ready (fill out). Then you have to create your own world, so in the lobby there will still be a look if that somehow doesn't work a YouTube tutorial

Yes, you can select creative mode as normal, and just go to your own map or the one you want
When I'm in creative mode then I have different worlds like I do in such a menu but I can't create my own. How does that work and whenever I go into creative mode I can only play with other people and never alone
If you chose creatively, you have two options to either create or play.
There you go to create and then you should be alone in creative mode.
And then there should actually be a yellow crack (portal) straight out of the spawn there you go in and then you can create your own island
