If I want to give someone a Fortnite Skin for my birthday and he has a PC, how does that work?

With a Paysafe Card.

Abbreviation is PSC and you can redeem the code and then stop the V-Bucks or whatever…

Or you can buy a code for a skin on cdkeys.com.

But you have to pay attention to which platform you buy it because you can redeem the code only on the respective platform.
